Myomectomy
A myomectomy is a surgical procedure performed to remove uterine fibroids, also known as myomas or leiomyomas. Fibroids are non-cancerous growths that develop in the uterus and can cause symptoms such as heavy menstrual bleeding, pelvic pain, pressure on the bladder or bowel, and fertility problems
Here’s an overview of the myomectomy procedure:
Preparation: Before the surgery, the patient will undergo a thorough examination, including medical history, physical examination, and possibly imaging tests such as ultrasound or MRI to locate and evaluate the fibroids. Blood tests may also be done to assess overall health and to check for anemia.
Anesthesia: Myomectomy can be performed under general anesthesia, where the patient is asleep during the procedure, or under regional anesthesia, such as epidural or spinal anesthesia, where only the lower part of the body is numbed.
Incision: The surgeon makes an incision in the abdomen or uses minimally invasive techniques like laparoscopy or robotic-assisted surgery. The choice of incision method depends on factors like the size, number, and location of the fibroids, as well as the surgeon’s expertise and the patient’s preferences.
Fibroid Removal: Once the uterus is accessed, the surgeon carefully identifies and removes the fibroids while preserving the healthy tissue of the uterus as much as possible. The method of removal depends on factors like the size and location of the fibroids. The surgeon may use techniques such as:
Enucleation: Cutting the fibroid out of the surrounding uterine tissue.
Morcellation: Cutting the fibroid into smaller pieces for easier removal, especially in laparoscopic or robotic surgeries.
Hysteroscopy: Removing fibroids that are located inside the uterine cavity using a hysteroscope inserted through the cervix.
Closure: After removing all the fibroids, the surgeon carefully closes the incision(s) with sutures. If minimally invasive techniques were used, the incisions are usually small and may require only a few stitches or adhesive strips.
Recovery: After the surgery, the patient is taken to the recovery room and monitored closely. Pain medication may be prescribed to manage any discomfort. The length of hospital stay varies depending on the type of surgery and individual recovery, ranging from a few hours for minimally invasive procedures to a few days for open surgery.
Postoperative Care: Following discharge from the hospital, the patient will be advised to take it easy and avoid heavy lifting or strenuous activities for a few weeks. Follow-up appointments will be scheduled to monitor recovery and address any concerns.
Myomectomy aims to alleviate symptoms caused by fibroids while preserving the uterus, which is particularly important for women who desire to maintain their fertility or avoid a hysterectomy. However, it’s essential to discuss the risks, benefits, and alternatives of the procedure with a healthcare provider to make an informed decision.
